2012-2013 NPHS Athletic/Activity Forms
Multiple forms are needed to participate in the New Prague Area Schools Activities/Athletics programs. The following forms must be turned in prior to the first practice in order to be eligible to participate. Official first date of practice is August 13, 2012.

1. 2012-2013 MSHSL Eligibility Form (PDF)
   • This form will cover all Athletic & Fine Arts sponsored by the MSHSL & ISD #721.
2. 2012-2013 MSHSL Physical Form (PDF) - completed by your medical doctor.
   • A "current" physical (within the last three years) must be on file with the NPHS Activities Department and indicates fitness for participation.
   • Please call the NPHS Activities Dept. at (952) 758-1226 (or 1227) if you have questions in regards to your son(s)/daughter(s) date of their last physical.
3. New Prague High School Medical Consent Form (PDF)
4. ImPACT Consent Form (PDF)
   • Necessary for all "contact" sports: football; cheerleading; soccer (B & G); swimming/diving; basketball (B & G); hockey (B & G); wrestling; & gymnastics.
