H1N1 Immunizations at New Prague Area Schools
Scott County Public Health will be returning to your child's school to provide second doses of H1N1, and to vaccinate those children age 10 and older whose parents had previously submitted consent forms for vaccination. If your under the age of 10 child was not vaccinated during the first clinic, you are still able to have the child vaccinated but you will need to have the follow-up (second dose) vaccination done at a different location. All families have received consent forms that must be completed, signed and returned to their school. The consent form can be downloaded from the District Website www.np.k12.mn.us and click on the H1N1 tab on the left side of the screen.

All children in the school district are eligible to receive the H1N1 vaccine if a completed consent form is turned into the school where they attend.

Dates and locations for the H1N1 vaccinations are:

January 8, 2010 Falcon Ridge Elementary School – 8:30 am start
January 8, 2010 Holy Cross School – 1:30 pm start

January 12, 2010 St. Wenceslaus – 8:30 am start
January 12, 2010 Eagle View Elementary School – 12:30 pm start

January 13, 2010 Raven Stream Elementary School 12:30 pm start

January 14, 2010 Middle School 8:15 am start

January 15, 2010 High School 8:15 am start
